We are delighted to share that our Palliative Care Hub is now a 24-hour service offering support to people living with an advanced serious illness or other life-limiting condition and those who need end-of-life care.
End of life care is vital support for people who are in the last months or years of their life. Specialist Palliative Care Clinical Nurses are now available around the clock, 24 hours a day, seven days a week as the Palliative Care Hub becomes a fully 24-hour service.
The service, which is commissioned by the Cambridgeshire and Peterborough Clinical Commissioning Group (CCG) and operated by Arthur Rank Hospice Charity in partnership with HUC, is for anyone living in Cambridgeshire or Peterborough, including The Fens, Huntingdon and Wisbech. It is for anyone living with a progressive or life-limiting condition. These can include dementia, heart failure, respiratory failure, neurological conditions, and more.
The Palliative Care Hub offers support and guidance. All the nurses are specialists in palliative care and are there to help with advice on matters such as symptom management and quality of life. The Hub will signpost to community services that are available. Most importantly the Hub nurses are also at the end of the phone to listen, and support when needed.
The specialist care nurses can help guide anyone living with a life limiting illness or condition, their family and friends with ongoing treatment, symptom management and any care needs. It also helps focus on the needs and wishes of the individual, when they are at the end of their life. It includes, but is not limited to:
- Managing symptoms that affect physical health such as pain, fatigue or breathlessness
- Ongoing psychological, emotional and spiritual support including faith and beliefs
- Practical care including physical needs like dressing, washing, and managing dietary intake
- Guidance and support for family and friends including coping with what to expect.
You can contact the hub by calling 111 and selecting the ‘Palliative and End of Life Care’ option to speak to a clinical nurse specialist. If you would like anymore information please call us on 01223 675777.
